Coding Help: Always use CPT 82274 (QW modifier is appropriate to denote a CLIA waived test). For Medicare or Medicaid, use g0328Q. If it was planned as screening visit, then it should be billed as screening only, not as diagnostic. Diagnosis does not turn it to diagnostic visit instead of screening visit. As per coding guidelines, you can bill screening ICD code followed by finding ICD codes. ICD Z12.11 can be used for; encounter for screening for malignant neoplasm of colon or encounter for screening colonoscopy NOS and R19.5 can be used for fecal abnormalities. On the other hand, some payers also suggest to bill abnormal finding first followed by screening codes.
